How to Begin Mental Health and Substance Use Services
Services begin with an intake assessment where you meet with one of our professional clinicians to review current concerns and personal history in order to best determine your individual service needs. A financial intake is also completed as part of this appointment. Please plan on spending 2-2.5 hours to complete the intake process.

For Adults
We offer intakes on a walk-in basis at the CSB Main Street location (1241 North Main St. Harrisonburg, VA 22802) on Mondays, Wednesdays, and Fridays from 8:00AM to 2:30PM. Please keep in mind intakes are provided on a first come first serve basis.
Please come in the main entrance and let clerical know you are seeking an intake appointment. They will provide you with an initial intake packet to complete and intake staff will meet with you to start the process. A copy of this paperwork can also be found on our documents page under “Intake Forms”. By clicking Adult Intake Packet below you should be able to complete the packet prior to arriving.
For Children
Currently we are scheduling intakes for children up to age 18. These intakes are held at the CSB Main Street location (1241 North Main St. Harrisonburg, VA 22802) on Tuesdays 9:00AM to 2:30PM and Thursdays 8:30AM to 2:30PM. A legal guardian and the child need to be present during the intake process.
To schedule, please call 540.434.1941 and ask to speak with intake staff. Staff will complete the initial intake packet with you over the phone and help you schedule an intake appointment for your child. A copy of this paperwork can also be found on our documents page under “Intake Forms”. By clicking Child Intake Packet below you should be able to fill out the packet to drop them off at our Main Street location, mail them, or fax them (Intake Fax: 540.433.3270). Once the paperwork is received, intake staff will contact you by phone to schedule an appointment.
Items to Bring to Your First Visit
- Insurance Card
- If you are covered by insurance, we will need your insurance card to be able to bill your insurance provider. If you have insurance and do not provide us with the information, you will be required to pay full fee.
- If the services you receive are not covered by your insurance or you do not have any insurance, then you can apply for a reduced fee.
- Verification of Household Income
- We need income information for all adults in the household to determine your eligibility for a reduced fee. This could include:
- Wages – recent check stub
- Retirement/Pension – annual report or bank statement showing direct deposit amount
- Disability – current disability papers
- Social Security – current social security papers
- Food Stamps – verification letter
- Child Support/Alimony – court order, bank statement showing direct deposit amount, or a copy of the check
- We need income information for all adults in the household to determine your eligibility for a reduced fee. This could include:
- Other Information
- Social Security Number(s) – for you and each adult members of the household
- Picture ID – your driver’s license or some other state issued identification, employment badge, student ID card, etc.
Not having the above information at time of intake will not prevent you from accessing services
