General Statement of Duties:
Assists case managers of clients with developmental disability by providing direct face-to-face client support and assistance to clients, providing transportation as needed, and completing some administrative support duties as needed. Exercises discretion and independent judgment in providing services to clients in homes and communities, in accordance with plans and instructions developed and communicated by case managers. Regular and on-going supervision is provided by the Supervisor of DD Case Management. Maintaining client confidentiality is a requirement.
This position is classified as non-exempt under the Fair Labor Standards Act.
Major Essential Functions:
Transport and accompany individuals to services and appointments.
Provides assistance to individuals in community settings or at their homes, with particular attention to health and safety.
Provide some assistance with Instrumental Activities of Daily Living, including transportation, cleaning, cooking, laundry, and managing finances.
Manage a varying schedule, which may include some evening hours.
Maintains frequent communication with the clients’ case managers regarding the clients’ needs and progress.
Keeps concise and current documentation of individuals’ progress and logs of services provided, using agency forms and procedures.
Assist with administrative duties which support the DD Case Management team
Reports regularly to supervisor.
Makes effective use of available technology, including computers, e-mail, and voice mail in order to enhance customer service.
Maintains effective relationships with co-workers and customers based on courtesy, compassion, and respect.
Performs duties as assigned consistent with established operational policies and procedures.
Required Knowledge, Skills, and Abilities:
Knowledge of developmental disability and/or related conditions; principles of record keeping and documentation; basic ethics appropriate to a helping relationship including treatment with respect, confidentiality, and human rights; demonstrated ability to work with individuals with developmental disability; ability to teach and assist clients in basic living skills; good interpersonal skills including the ability to empathize, demonstrate respect, communicate warmly and honestly, and allow a client to assume responsibility for himself/herself; honesty; good judgment.
Additional requirement:
Possession of a current, valid driver’s license and acceptable driving record.
Minimum Education and Experience:
Completion of an associate degree or two years of college in a human services field; OR completion of high school and one year of direct care experience with individuals with mental illness, developmental disability, or substance abusers; OR any equivalent combination of experience and training which provides the required knowledge, skills, and abilities.

HIPAA Access Level 2: Full read/write access to protected health information (PHI), limited to the minimum necessary, for all clients directly served or assigned, as well as all clients indirectly served through case consultation, clinical supervision, program management, or coverage requirements. Staff at Level 2 are allowed to seek out client PHI as necessary to perform their assigned duties but are expected to exercise due precautions to limit exposure to any PHI which is outside the scope of their need to know. Utilization of information will be in accordance with HIPAA regulations regarding use limitation, disclosure and requests of PHI.
